The memo says its rates reflect the 2025 fee schedule and to always check the current schedule; memo-sourced rows are labeled. Conflicts: 97151 is $32.13 on the 06/2026 schedule vs $28.52 (HO) in the memo; 97156 is $8.61 on the 06/2026 schedule vs $46.20 (HO) / $25.01 (HN) in the memo. Because 97156 differs this much, its rate is null. Modifiers: HO = Licensed Behavior Analyst (BCBA/BCBA-D), HN = BCaBA, HM = RBT; UN/UP/UQ/UR/US = 2/3/4/5/6+ members or families. 97153 and 97155 may be billed concurrently with different modifiers.
