How we read this state's schedule
---------------------------------

Max fee per 15-minute unit for DOS on/after 4/1/2026 (6% cut on individual codes; group codes re-tiered by group size with U4=2, U6=3, U8=4-8). A further 4% cut applies to all ABA codes for DOS on/after 4/1/2027. Later 2026 ABA bulletins (BT202646, BT202662, BT202692) do not change rates. Not cross-checked against the IHCP Fee Schedule web app (behind CPT click-through). UA modifier is appended for comprehensive ABA (not a rate tier). Coverage limited to EPSDT (<21) for DOS on/after 10/1/2026. MCE rates may differ.
